To access the Planning Center click on the Admin tab and scroll to the CQM-eCQM Planning Center link.

Note: Users with Master ACO access will have the ability to make updates to the table. All other users will have view only access.

Use this tool to track your practice's ability to report MIPS CQMs or eCQM Quality Measures.

This page will include all TINs from your Network Manager. As you make changes to your Network Manager to add/remove TINs this page will automatically update accordingly.

Note: Making changes on this page will NOT update your Network Manager they will only affect this page.

To make a change to a specific Practice in the grid click on the 'Edit' button located in the far left column in each row.

Once you have completed your changes click on the 'Update' button. Or, if you decide not to make changes you can click on the cancel button to go back to the original answers. 

The top of the grid has two buttons:
 

  • Export to Excel: This button will export the entire grid to an Excel file for review. 

  • Add Record: Clicking on this button will add a new row to your grid. However, this will NOT add this record to the Network Manager.

The next columns of the grid are static columns and will always be shown as you scroll:
 

  • ACO ID: Auto populated field with your ACOs MSSP ID Number

  • Reporting Options: Choose options from the dropdown to identity which reporting options the TIN is ready to report on. (CMS Web Interface, MIPS CQM, eCQM, Medicare CQM)

  • Practice Status:  Choose Active or Inactive from the dropdown. Practices that are removed from the Network Manager will automatically be set to Inactive in the Planning Center.

  • Practice Name: Free text field to enter/update Name of the practice. (Does not change Network Manager)

  • Practice TIN: Free text field to enter/update TIN number for practice

These columns relate to contacts and EHR information:
 

  • Primary Quality Contact: Free text field to enter the Primary Contact at the practice level.

  • Primary EHR Contact: Free text field to enter the Primary EHR Contact at the practice level.

  • EHR Vendor:  Free text field to enter EHR vendor for the selected practice.

  • EHR Version: Free text field to enter EHR Version.

  • ONC 2015 Edition Certified: Select Yes/No from the dropdown.

The next set of columns relate to confirming if the Practice EHR can generate appropriate files:
 

  • Can practice generate performance year QRDA I files?: Choose Yes/No from dropdown. There is also a link to download our QRDA requirements document. 

  • Can practice generate CMS WI Flat File?: Choose Yes/No from dropdown. There is also a link to download the CMS WI Measure specs.

  • Can practice generate MIPS CQM All Patient /Payer Flat File?:  Choose Yes/No from dropdown. There is also a link to download the MIPS CQM Measure Specs.

  • Can practice generate flat file for Medicare CQM?: Choose Yes/No from dropdown. Once measure specs are available there will be a link to download.

The remaining columns relate to training and completing file transfers:
 

  • Quality Video Education Completed: Choose Yes/No from dropdown to confirm if your practice has completed our online training video(s). 

  • Client complete SFTP form: Choose Yes/No from dropdown. There is a link to download our SFTP form if the answer is No.

  • SFTP Setup Completed:  Choose Yes/No from dropdown to confirm that the practice has successfully complete the SFTP setup process..

  • TIN sent MIPS CQM All Patients/Payer Flat File to SFTP: Choose Yes/No from dropdown to confirm that the practice successfully transferred their all patient/payer data via SFTP.

  • TIN send QRDA I via SFTP: Choose Yes/No from dropdown to confirm that QRDA I files have been sent via SFTP.

  • Education First Meeting: Enter date to confirm that the TIN has completed their first education meeting with Health Endeavors.

  • Most recent Meeting: Enter date of most recent meeting the practice has had with Health Endeavors. 

  • Comments: Free text to enter any notes that would like to track for each practice.
