PAC VRDC Executive Dashboard

Overview

The PAC Dashboard gives ACO stakeholders visibility into network performance across Home Health and Skilled Nursing Facilities, with dedicated tabs for each.

The Executive Dashboard provides a high-level view of performance across Home Health and Skilled Nursing Facilities (SNFs). It is designed to help you quickly compare facilities, groups of facilities, and network performance using key quality, utilization, and patient outcome measures.

The dashboard allows you to create two separate filter sets and compare the results side by side.

For example, you can compare:

  • One facility to another facility

  • One facility to a group of facilities

  • One group of facilities to another group of facilities

  • In-network providers to out-of-network providers

  • Different geographic or organizational groups

The Executive Dashboard is intended to provide a high level view of performance and help identify areas for further review.

Selecting Your Care Type

At the top of the dashboard, select the type of care you want to review:

  • Home Health

  • Skilled Nursing Facilities (SNFs)

The graphs and measures displayed will change based on your selection.

Filters

Using the Filters

The Executive Dashboard uses two sets of filters, allowing you to compare two different groups of facilities or providers.

Each filter set includes the following selections:

  • Data Period

  • Network Period

  • Location

  • Network

  • Facility

Selections made in each filter set determine the data displayed for that comparison.

This flexibility allows you to compare performance in the way that is most meaningful to your organization.

Data Period

  • The Data Period determines the period of performance data being displayed.

  • Data Periods currently extend through Q4 2025. As new data sets become available, additional quarters will be added to the dashboard.

  • Data displayed represents a rolling 12-month period ending with the selected quarter.

    • For example, selecting Q4 2025 displays data representing the rolling 12 months through Q4 2025.

  • The available Data Periods will be updated over time as new data becomes available.

Network Period

  • The Network Period determines which version of your organization's network is used for the comparison.

  • Network information is based on the in-network file provided by your organization. The Network Period dropdown reflects what your organization's network looked like during the selected period.

Data Period and Network Period serve different purposes:

  • Data Period determines the period of performance data being reviewed.

  • Network Period determines which facilities were considered in network during that period.

Location

The Location filter allows facilities to be grouped according to the structure that makes the most sense for your organization.

Locations may be based on geography, market, region, or another organizational grouping.

Examples could include:

  • North Carolina

  • South Carolina

  • Las Vegas Area

  • Charlotte Area

These groupings are configured based on your organization's needs and can be modified as those needs change.

Network

The Network filter allows you to choose which providers or facilities to include:

  • In Network

  • Out of Network

  • All (This includes both in network and out of network)

Facility

Once the Data Period, Network Period, Location, and Network selections are made, the Facility dropdown will populate with the applicable facilities.

Use the Facility dropdown to select one or multiple facilities for your comparison. Selecting No facility (Leaving the dropdown blank) will automatically select all facilities available in the dropdown.

Understanding the Data

The graphs on the PAC VRDC Executive Dashboard display performance based on your selected filters.

Unless otherwise specified, the data represents a rolling 12-month period through the selected Data Period.

Data displayed on the dashboard is based on CMS data and incorporated into the dashboard.

The dashboard provides different measures depending on whether you are viewing Home Health or Skilled Nursing Facilities.

Home Health

When Home Health is selected, the Executive Dashboard includes the following measures:

  • Admissions

  • HHCAHPS Summary Star Rating

  • Timeliness of Care

  • Risk Score (V28)

  • Quality Star Rating

  • Rehospitalization Rate

    • This measure also includes benchmark information that allows you to compare the                                   percentage of providers meeting the benchmark between your two filter sets.

  • Emergency Room Utilization

    • This measure also includes benchmark information that allows you to compare the                                   percentage of providers meeting the benchmark between your two filter sets.

  • Preventable Rehospitalization Rate

    • This measure also includes benchmark information that allows you to compare the                                   percentage of providers meeting the benchmark between your two filter sets.

  • Avoidable Emergency Room Utilization

    • This measure also includes benchmark information that allows you to compare the                                   percentage of providers meeting the benchmark between your two filter sets.

Skilled Nursing Facilities

When Skilled Nursing Facilities (SNFs) are selected, the Executive Dashboard includes the following measures:

  • Admissions

  • Risk Score (V28)

  • Quality Star Rating

  • Overall Star Rating

  • Average Length of Stay

  • Rehospitalization Rate

    • This measure also includes benchmark information that allows you to compare the                                 percentage of providers meeting the benchmark between your two filter sets.

  • Preventable Rehospitalization Rate

    • This measure also includes benchmark information that allows you to compare the                                 percentage of providers meeting the benchmark between your two filter sets.

  • Emergency Room Utilization

    • This measure also includes benchmark information that allows you to compare the                                 percentage of providers meeting the benchmark between your two filter sets.

  • Avoidable Emergency Room Utilization

    • This measure also includes benchmark information that allows you to compare the                                 percentage of providers meeting the benchmark between your two filter sets.

Understanding Benchmark Graphs

Four measures on the Executive Dashboard include additional benchmark information for both Home Health and SNF:

  • Rehospitalization Rate

  • Emergency Room Utilization

  • Preventable Rehospitalization Rate

  • Avoidable Emergency Room Utilization

These graphs provide more than the performance value alone. They allow you to compare benchmark performance between the two filter sets and explore the providers contributing to the results.

How to Read the Graphs

The data is shown in a bar chart with two color-coded groups.

At the bottom of each graph, there is a legend where you can see which filter sets correspond to which bar color.

The percentages presented in the graphs are based on the facilities that meet the filters selected for each data set. Because the filters determine which facilities are included, changing the filters will also change the data displayed.

Difference Percentage: Shows how far apart the two percentages are compared with their average. We find the difference between the two, then compare that difference to the average of the two percentages.

Providers Meeting Benchmark

The Providers Meeting Benchmark percentage shows the percentage of providers within the selected group that meet the applicable benchmark.

Because the Executive Dashboard allows you to select two filter sets, a benchmark percentage is displayed for each comparison group.

For example:

Filter Set 1: 46.26% of providers meet benchmark
Filter Set 2: 44.75% of providers meet benchmark

This provides a quick way to compare benchmark performance between the two groups.

Patients Going to Providers Meeting Benchmark

The benchmark graphs also provide information about patients going to providers who met the benchmark.

This provides an additional perspective on benchmark performance by showing the patient population associated with providers meeting the benchmark.

Benchmark Graph Time Periods

Benchmark graphs display two to three time periods:

  • 7 day

  • 30 day

  • 60 day

The graph also displays a 30-day benchmark line for each filter set.

The benchmark line applies specifically to the 30-day benchmark.

When reviewing the graph, compare the 30-day data point with the corresponding benchmark line.

The 7-day and 60-day values provide additional context but are not being compared against the 30-day benchmark line.

Moving From the Executive Dashboard to the Management Dashboard

The first way to move from the Executive Dashboard to the Management Dashboard is to select the PAC VRDC Management Dashboard from the Analytics dropdown and looking under the Interactive column.

The second way is to look at the Providers Meeting Benchmark percentages on applicable graphs.

These data sets are interactive.

Selecting one of these percentages takes you to the Management Dashboard for a more detailed analysis.

The applicable selections from the Executive Dashboard are carried forward, including the relevant:

  • Data Period

  • Network Period

  • Network

  • Facility selections and/or Location(Region) selections

The benchmark selection is also carried into the Management Dashboard.

Example

Suppose you are reviewing Rehospitalization Rate and have selected two comparison groups.

If you select the Providers Meeting Benchmark percentage for Filter Set 1, the Management Dashboard will open with the applicable filters from Filter Set 1 and the Rehospitalization Rate benchmark criteria applied.

This allows you to move directly from the high level Executive Dashboard result to the providers included in that benchmark result.
