  • Medicare ACO CCLF

    • Medicare ACOs receive a  monthly claims data feed for all patients assigned to their ACO.​  Your organization must apply to and be approved to participate in this program.

  • Medicare Data Point of Care (DPC) Bulk FHIR

    • Medicare DPC Bulk FHIR​ claims data is available to all healthcare providers that have a treatment related purpose to view a patient's health history.  It is updated every 3 weeks.  Providers must request access to the data and receive access keys to participate.

  • Medicare Blue Button Individual FHIR

    • Medicare Blue Button FHIR claims data is individually approved by a Medicare patient to share with their providers, family, insurance agents or others.  It is updated every 7 days.  Only approved vendors such as Get Your Health Record may store and share the health information with the patient's consent.

  • Medicare Advantage

    • Healthcare providers request Medicare Advantage claims data from the payers.

  • Medicaid

    • Healthcare providers request Medicaid claims data from the state Medicaid agencies.​

  • Commercial 

    • Healthcare providers request commercial claims data from the payers in FHIR EOB or flat file format.​

  • Self-Insured Employer Third Party Administrator (TPA)

    • Healthcare providers request TPA claims data from either the TPA or employer.​

​​

  • QRDA

    • QRDA is a quality measure architecture for quality data feeds.​

​​

  • FHIR

    •  FHIR (Fast Healthcare Interoperability Resources) Specification, is a standard for exchanging healthcare information electronically.  We use Smart on FHIR to deliver data to the EHR and to pull data ("Get Calls") from the EHR.

  • CCDA

    • ​Consolidated Clinical Document Architecture. CCDA is a complete architecture used to create documents and template methodologies for medical documents. Primary function of the CCDA is to standardize the content and structure for clinical care summaries.

  • Remote Patient Monitoring (RPM)

    • Daily device recordings, trends and poor results received from remote monitoring devices.

  • Social Determinants of Health (SDOH)

    • SDOH data collected from patient online questionnaires.​

  • COVID-19 Symptom Screening

    • Screening responses collected from patient online questionnaires.​

  • Interactive Health History Form

    • Data collected from patient responses to interactive health history form populated with their claims history.  Responses such as Taking, Not Taking, Not Aware of Diagnoses, Managing Diagnoses, or Not Managing Diagnoses.​

  • Admit, Discharge, Transfer (ADT)

    • Events-based notification of admit, discharge or transfer for a patient. ​

​

  • Lab Values

    • HBA1C, LDL-C, etc. lab values and dates of services via HL7 data feed from lab company.​
